DayBreak is an adult day health program that offers a safe and stimulating environment with therapeutic activities, socialization, exercise programs as well as medical and personal care services for seniors. It provides caregivers an important day time respite that allows them to continue their employment, run errands or simply recharge. DayBreak is a fun, safe and uplifting environment and is the only provider of these services in our area.
The In-Home program provides house management, housekeeping, meal planning and preparation, shopping, chore services, health screening and up-to-date education regarding senior health issues. The In-Home program also provides socialization for our clients, many of whom are isolated due to physical limitations or a rural location. This program serves low income seniors.
Care Management helps seniors remain safely in their own homes. Many seniors, especially those that are low income or isolated due to a rural address, need an advocate that can provide information and assistance in order to assure that all available resources are utilized and to maintain a safe home environment. This program allows seniors to delay nursing home placement.
